How should a face-lift plan address bleeding, haematoma and facial-nerve risk?

Bleeding and facial-nerve weakness are recognised facelift risks. Skin-only, SMAS and deep-plane surgery involve different tissue planes, with the nerve exposed differently in each, so the intended plane should be named in writing. Haematoma is the complication most likely to require return to theatre: a 2001–2013 meta-analysis reported expanding haematoma in roughly 1.8% of cases and the TOPS database about 1.97%; approximately 90% appear within 24 hours. Small collections may distort contour, whereas large ones can endanger a skin flap and, rarely, the airway. Meaningful collections require finding the bleeding source and evacuation; opening sutures to decompress has been described only as a temporary measure if evacuation is delayed. Poorly controlled blood pressure is a key risk. Facial-nerve transection is uncommon. Weakness persisting after the anaesthetic can reflect a suture, traction or cautery heat and may recover over days or weeks; weakness that does not improve needs review by the operating team. What should the written plan show about facial and chin incisions?

Ask to see the planned incision path through hair-bearing scalp, before the ear, behind the lobule and into the hair behind the ear. ASPS identifies unfavourable scars, temporary or permanent hair loss at an incision, asymmetry and visible distortion at its ends as facelift risks. Excess pull on the lobule can create a forward-drawn “pixie ear”. For chin surgery, the incision may be intraoral or placed in skin beneath the chin. The intraoral route avoids an external scar, but infection and implant contact with oral bacteria still need separate discussion; a submental approach leaves a short external scar. Which symptoms need urgent assessment after facial surgery or facial fat injection?

The warning signs depend on the procedure. After a face, neck, brow or temporal lift, seek urgent assessment for rapidly enlarging one-sided swelling, pain, breathlessness or new facial weakness. After eyelid surgery, a non-closing lid, intense pain, a protruding eye or poorer vision requires prompt review. Sudden visual loss following facial fat injection is an eye emergency; fat within a vessel also needs emergency ophthalmic assessment. ASPS describes blindness after eyelid surgery as very rare. A clinically important haematoma needs evacuation. The NHS recommends agreeing in writing, before treatment abroad, where review occurs, the expense of further surgery and return travel, and insurance cover; travel insurance usually excludes surgical complications. The NHS uses 7–10 days after facial cosmetic surgery as a flight-planning interval, while the operating surgeon decides individual clearance.
