Bleeding and facial-nerve weakness are recognised facelift risks. Skin-only, SMAS and deep-plane surgery involve different tissue planes, with the nerve exposed differently in each, so the intended plane should be named in writing. Haematoma is the complication most likely to require return to theatre: a 2001–2013 meta-analysis reported expanding haematoma in roughly 1.8% of cases and the TOPS database about 1.97%; approximately 90% appear within 24 hours. Small collections may distort contour, whereas large ones can endanger a skin flap and, rarely, the airway. The warning signs depend on the procedure. After a face, neck, brow or temporal lift, seek urgent assessment for rapidly enlarging one-sided swelling, pain, breathlessness or new facial weakness. After eyelid surgery, a non-closing lid, intense pain, a protruding eye or poorer vision requires prompt review. Sudden visual loss following facial fat injection is an eye emergency; fat within a vessel also needs emergency ophthalmic assessment. ASPS describes blindness after eyelid surgery as very rare. A clinically important haematoma needs evacuation.
