The nasal tip sits at the front of a flexible framework that influences profile, nostril shape, the angle between the nose and upper lip, and the stability of the external nasal valve. In nasal tip support rhinoplasty, surgeons use anatomical analysis rather than a universal template. Terms such as projection, rotation and deprojection are useful ways to describe direction of change, but none is a promise of a particular angle, millimetre measurement or appearance for an individual.
Why the tip needs support
The tip is primarily shaped by the paired lower lateral cartilages. Each cartilage has a medial crus near the columella, an intermediate segment that helps form the dome, and a lateral crus extending toward the nostril sidewall. These parts do not act alone. The septum provides a central reference and support point; the scroll region connects the lower and upper lateral cartilages; and soft tissue, ligaments and skin help hold the framework in position.

Support has an aesthetic and a functional dimension. The lower lateral cartilages and adjacent soft tissue help form the external nasal valve—the entrance region of the nostril. The classic tripod theory makes tip mechanics easier to visualise. It is a memorable teaching model because it links a cartilage manoeuvre to a likely directional effect.
For example, reducing the cephalic portion of the lateral crura or changing their position may influence apparent rotation and projection. Altering the medial crura or their relationship with the caudal septum may influence tip position and columellar show. But a living nose is more complicated than a rigid tripod. Cartilage has variable stiffness and curvature; skin can be thin, thick or scarred; ligaments and healing forces matter; and a manoeuvre on one side may have a different effect in an asymmetric nose. The model should therefore guide anatomical thinking, not replace it.
This is also why online statements such as “one suture always rotates the tip” are unreliable. Tip projection describes how far forward the nasal tip sits in relation to the rest of the face and nose, especially in profile. A more projected tip appears to come forward; a less projected tip appears closer to the face. Projection is not synonymous with size. A nose can look long because of rotation, dorsal height, columellar position, chin projection or facial proportions even when the actual tip projection is moderate.
Clinicians use profile photographs, frontal and base views, direct examination and established proportional references to assess projection. Those references are planning aids rather than beauty laws. They were often developed from limited populations, and no single ratio captures a person’s ethnicity, sex, age, skin quality, facial skeleton or goals. A numerically “ideal” projection can look out of place on a particular face; a proportion that falls outside a textbook range can look entirely harmonious.
When a tip lacks structural support, it may look underprojected, broad, droopy or poorly defined, but those descriptions can also arise from a low bridge, thick skin, short columella, weak cartilage, asymmetry or a rotated tip. Rotation: the direction the tip points
Tip rotation describes the upward or downward orientation of the tip. In profile, it is often discussed through the nasolabial angle, the angle formed by the columella and upper lip. In everyday language, increased rotation can make the tip look more upturned, while decreased rotation can make it point more downward. Neither direction is inherently better.
Rotation should be judged with the entire face in view. The upper lip, columella, nostril show, bridge, chin and smile dynamics all affect what looks balanced. A small increase in rotation may reveal more nostril from the front or profile; too much can create a result that looks over-rotated rather than natural. Too little rotation may leave a tip appearing heavy or downturned in some anatomy, but a more conservative position may suit other faces. Exact-angle promises are therefore a warning sign, not a mark of precision.
Dynamic assessment matters as well. Muscles around the nose and upper lip can change tip and columellar movement during smiling. Previous surgery, trauma, scarring and cartilage memory can make the same technical manoeuvre behave differently from a primary, unoperated nose. The appropriate discussion is about the desired direction of change and its tradeoffs, not a guarantee that a particular photo or angle can be reproduced.
What does deprojection mean?
Deprojection means reducing the forward prominence of the tip. It is commonly misunderstood as simply “pushing the tip back.” In practice, it can involve changing cartilage relationships, shortening or repositioning selected structures, modifying a projection-supporting framework, or balancing the tip with the bridge and columella. Because these structures contribute to shape and valve stability, deprojection must be considered with particular caution.
A deprojected tip can look more balanced in selected anatomy, but excessive loss of support may produce an unnatural contour, alar changes, columellar retraction or breathing concerns. The direction of effect is not mechanical certainty: soft tissue can contract, scar tissue can pull, and healing can reveal asymmetry that was not obvious early on. For that reason, “make it much smaller” is not an anatomical plan. The question is which structure creates the perception of excess projection and what can be altered while preserving a stable nose.
How support is preserved or reinforced
Tip surgery can involve conservative cartilage reshaping, sutures that alter dome relationships, repositioning of cartilage segments, or selected grafts. The purpose may be definition, symmetry, projection control, rotation control, valve support, reconstruction or a combination of these goals. The intervention should follow the diagnosed anatomical problem; the name of a graft does not, by itself, establish a better operation.
A columellar strut is one familiar support concept. It is placed between the medial crura to help stabilise their relationship. A septal extension graft is attached to the septum and can provide a more fixed platform for controlling tip position in selected cases. These are not interchangeable labels: their design, source material, fixation, stiffness and effect on the tip can differ. The revision literature is especially relevant because prior surgery may leave distorted cartilage, adhesions, weakened support or limited donor material. Bulbous tips, skin and support are related—but not identical
A bulbous-looking tip may be associated with wide, convex, weak or asymmetric lower lateral cartilages. It may also reflect thick skin, soft-tissue fullness, limited projection, bridge-to-tip imbalance or several factors together. This is why a “bulbous tip technique” cannot be selected from a photograph alone. Removing cartilage without understanding the support system can trade width for weakness or irregularity.
For some noses, controlled support and repositioning produce a more coherent change than maximal cartilage removal. For others, skin thickness limits the amount of fine definition that will be visible even when the framework is carefully shaped. Approach does not replace tip-support planning
Open and endonasal approaches are ways of reaching the framework; neither is a standalone tip-support technique. Open exposure can make detailed visualisation and graft placement more straightforward in some complex tip, asymmetric or revision cases. Endonasal surgery can also include selected tip sutures, cartilage modification and grafting in experienced hands. The best approach is the one that allows the planned structures to be addressed safely and controllably for the particular anatomy.
